Personal Data Sharing Consent Form

I have been informed that personal data I have provided for diagnostic and treatment services, as well as any personal information obtained as a result of healthcare services, are required to be transmitted to the Ministry of Health under the USVS 2.0 system starting from January 1, 2013, and that healthcare providers and institutions are obliged by the Ministry of Health to do so.

I have been informed that the requested information may include: all identity, address, contact details, pregnancy test results, medical history, disability status, marital status, alcohol-drug-smoking use, occupation, education level, income status, complaints, patient history (anamnesis), all test results, institutions where tests were requested, birth and miscarriage details for women aged 15–49, women’s health procedures, family planning methods used, pregnancy detection results, date of last menstruation, father’s blood type, whether pregnancy resulted in birth or miscarriage, all dental and oral health-related preventive, diagnostic, and treatment services, and many other details.

I understand that these personal records requested by the Ministry of Health are considered sensitive personal data and that sharing them with individuals or institutions other than the healthcare professionals providing diagnosis, treatment, or tests without my explicit consent would be a violation of my right to privacy and the protection of my personal data.

I was informed that Article 20 of the Constitution of the Republic of Turkey states: “Everyone has the right to request the protection of their personal data. This right includes being informed of, accessing, and requesting the correction or deletion of their data. Personal data can only be processed in cases provided by law or with the explicit consent of the individual.” and that the procedures and principles regarding personal data protection are regulated by law.

I was informed that the Ministry of Health will create a personal health data record with the collected information, that the collected data will be anonymized for analysis purposes, and the results will be used for planning healthcare services and forming health policies, and that the Ministry does not require patient consent for this data collection process.

I was also informed that no guarantees are provided to me regarding the security of my data after it is collected and processed in this manner. No explanations were made regarding who or which institution would be responsible in case of any potential issues or violations.

In light of all this information, I hereby give my consent for the transmission of my personal data, whether provided by me or obtained during the healthcare services, to the Ministry of Health within the scope of diagnostic and treatment services.
